Referrals can be made a number of ways:
Directly by yourself by calling (07 5449 0008) or email (email@example.com) or via this website here
Through your disability support provider
or through the NDIS portal (we are called J.H MCMANIS & T RILEY trading as All Abilities in Noosa and Gympie)
Once we have received your referral or request for service you will be asked to complete an information sharing consent form:
Once we have received this form from you, and your current contact details, we will arrange a time for you to meet with one of our clinicians in our clinic or somewhere else mutually agreed (i.e. your home, support provider office, or school) to discuss, finalise, and sign of on the service agreement using the following form:
Once the service agreement is signed, it will be uploaded to NDIS and a service connection will be made between the NDIS participant and All Abilities within the portal as per the service agreement. This will then allow services to be delivered and payment obtained.
If you are self managing your NDIS funds
The above process is not required. Instead, as if you were paying for services privately, you can make appointments yourself by calling (07 5449 0008) or email (firstname.lastname@example.org) or via this website here, and we will invoice you or NDIS directly depending on how you've arranged self-managing payments to be processed. We can accept payment vis EFTPOS, credit card, or direct deposit.
